Job Responsibilities:

• Design, document, implement, and monitor care plan services for the client and support in a timely manner.
• Periodically monitor the status of implementation of care plan needed for all Client assigned to the caseload, completing required case documentation consistent with current agency and/or state standards, e.g. Periodic Progress Reports, Client Development Evaluation Reports, case notation, etc.
• Provide support and guidance to Clients, family members, and/or guardians or conservators regarding developmental disabilities, services, and supports that may assist the Client and/or family to achieve their desired goals and objectives.
• Assist clients and/or their families to access publicly available resources (e.g. Medical, schools, Mental Health, transit, etc.) and to identify naturally available resources in their lives and communities that may assist them to live a full and inclusive lifestyle.
• Collaborate with and/or consult with the agency or external multi-disciplinary resources to assist clients in identifying their needs and in determining plans to meet their needs.
• When necessary, make referrals to vendors/service providers to meet the Client’s needs. Research and provide information and referral to generic services and resources in the community, including the following but not limited to:

1. schools, recreational options, and local support groups.
2. supportive employment/habilitation-funded day programs.
3. local agencies
4. specialist dentists/physicians/ specialists (Speech Therapist, O.T./P.T.).

• coordinate and government-funded services, including:

1. behavior services, training for independent living, supportive living.
2. out-of-home placement, in home and out-of-home respite, adult day programs.
3. transit/transportation services

• assisting in resources for durable medical equipment and specialized services, non MediCal-funded services.
• Design, document, implement, and monitor care services for the client and support in a timely manner.
• Schedule appointments and check-ups necessary to ensure proper generic care is received on a timely basis.
• Conduct strength-based assessments that communicate an understanding of the unique issues facing family members.
• Involve individuals we serve and family in all aspects of person-centered planning and support activities.
• Facilitate planning team progress, identifying outcomes, needed services and support.
• Provide relevant information and training about targeted services, and resources.

Job Requirements:

• Minimum qualifications require a Bachelors-level degree in the human services field of study (Nursing, Psychology, or any medical course).
• At least 3 years of experience in the medical field, senior care and developmental disabilities service delivery.
• Knowledge in Google Suite applications (Sheets, Docs, Slides, Forms)
• Quick learner and ability to adapt to a rapid changing environment.
