Role: Care Coordinator

Work Arrangement: Fully remote, overlapping PST working hours

Job Type: Independent Contractor, Full-time

Work Schedule:

Shift A: 5:00am - 2:00pm PST (with 1 hr break) Mon - Thurs + Sun

Shift B: 11:00am - 8:00pm PST (with 1 hr break) Mon - Thurs + Sat

Locations: Remote

Role Overview:

The Care Coordinator will play a pivotal role in ensuring the clients receive exceptional care and seamless service. You will serve as a primary point of contact for clients, facilitating their health journeys, coordinating care plans, and ensuring all aspects of their experience are handled with precision and professionalism.


Core Responsibilities:

  • Client Relationship Management:
  • Serve as the first point of contact for clients, providing prompt and empathetic responses to inquiries.
  • Develop and maintain strong relationships with clients to ensure high satisfaction and trust.
  • Guide clients through onboarding, health assessments, and care plan implementation.
  • Care Plan Coordination:
  • Collaborate with healthcare providers and Superpower's internal teams to create and update personalized care plans for clients.
  • Monitor client progress and proactively address any concerns or adjustments needed in their care plans.
  • Coordinate appointments, follow-ups, and communications between clients and healthcare providers.
  • Operational Support:
  • Maintain accurate and up-to-date client records in internal systems.
  • Streamline processes to enhance efficiency in care coordination.
  • Handle scheduling and logistics for client consultations and services.
  • Data Management and Reporting:
  • Track and analyze client health metrics to identify trends and opportunities for improvement.
  • Generate reports on client outcomes and service effectiveness for internal review.
  • Ensure compliance with privacy regulations and confidentiality standards.
  • Educational Support:
  • Provide clients with resources and education to support their understanding of their care plans.
  • Stay updated on health and wellness trends to offer relevant recommendations to clients.

Requirements

  • Minimum of 3+ years of experience in healthcare coordination, client management, or a related field.
  • Background in the health and wellness industries is highly preferred.
  • Exceptional communication and interpersonal skills, with the ability to empathize and build trust.
  • Strong organizational and time-management abilities.
  • Tech-savvy and comfortable using healthcare platforms and CRM systems.
  • Proactive, solutions-oriented mindset with a commitment to excellence in client care.
